Contact Us

You can request an appointment or submit a referral by phoning us, emailing, or using the contact form on this page. For professional referrals (e.g. from an attorney or insurer), please include the referral question and any relevant documentation. We will confirm scope, availability, and next steps.

It depends on the type of service. Some assessments are requested by attorneys, insurers, or employers who will provide the referral. For individual clients seeking occupational therapy or assessment, a referral may not always be required. Contact us to discuss your situation and we will advise what is needed.

We will advise you when your appointment is confirmed. Generally, bring any relevant medical or legal documentation you have been asked to provide, a form of identification, and comfortable clothing if the assessment involves physical tasks. Specific requirements depend on the type of assessment.

We handle all client information in line with professional confidentiality standards and applicable law. Information is used only for the purpose of the assessment or service and is shared only with authorised parties as agreed (e.g. the referring party). We can discuss confidentiality and data handling in more detail when you make an enquiry.
